Does the documentation contain a definitive list of all supported
markup anywhere? It appears that __foo__ underlines foo (which it
didn't do before), and this isn't noted anywhere that I can find in
the documentation. Nor it notated anywhere how to display the markup
as literal text in-line (i.e., without the 'preformatting' style,
which isn't what you want in many cases).
